Application and Affidavits for Septic installation:

  • Our company installs septic systems in the following cities and counties
  • Application must be filled out by the property owner
  • Application and affidavit must be signed
  • Application must have the applicant's driver's license number
  • Affidavit must have legal description (Lot, block, subdivision, etc...)
  • Affidavit must be notarized 
  • Original documents must be filed (Copies will not be accepted by the county)
