Application and Affidavits for Septic installation:

  • Our company installs septic systems in the following cities and counties
  • Application must be filled out by property owner
  • Application and affidavit must be signed
  • Application must have drivers license number of applicant
  • Affidavit must have legal discription (Lot, block, sub-division, etc...)
  • Affidavit must be notarized 
  • Original documents must be filed (Copies will not be accept by county)
